Auto/Property Damage Claim Procedure


  1. Download and complete the “SWORN STATEMENT AND PROOF OF LOSS” form detailing date, time and specifics of incident or police report. Be sure to have the form NOTARIZED before submitting.
  2. Also complete the “SCHEDULE OF ARTICLE/PROPERTY DAMAGE” form and submit estimates of damages or related receipts.
  3. Copy of "declaration of insurance" summary page on home or vehicle listing the deductible and limits of coverage.

All three above items must be submitted to the LAW DEPARTMENT OFFICE, 200 West Erie Avenue 7th Floor. The claims will be reviewed and investigated by the legal department. Correspondence from the legal department will be forwarded to the claimant and to Lorain City Council advising of recommended approval or denial.
